School Linked Services
To support children and their caregivers throughout our community who need mental health services, we provide a strength-based service within local schools. School case managers work closely with the student, their family and the student’s support team (e.g., educators, therapists, childcare providers, etc.) to build relationships and implement an individualized care plan. School case managers are committed to meeting with the student and/or their families at school, home, daycare, or any other location that is convenient for the student and their family.
School Linked Services eligibility
To qualify for this service, the student must be Medicaid eligible, display a significant functional impairment, and have a mental health diagnosis.
This service is a unique collaboration between Nexus Family Healing and school districts through a formal MOU. The case manager is employed by Nexus but works at the school, essentially becoming part of the school staff.
