Opening in August 2026. Located in Eugene, Oregon, our program offers a safe and supportive setting in which each youth has their own bedroom, along with spacious areas designed for therapeutic services, enriched education, life-skills development and daily recreational activities.


Programs offered
Nexus programs are designed to keep families engaged throughout the treatment process—and to ensure wrap-around support is needed during this difficult time. As youth develop new skills and coping strategies, families receive the tools and support needed to foster healing, resilience, and long-term stability.

Psychiatric residential treatment facility
Ages 12-17 Males and females
Our Psychiatric Residential Treatment Facility (PRTF) in Eugene is a secure and trauma-responsive facility for youth ages 12-17 who are navigating mental health and development challenges and have involvement with the juvenile justice system.
Our program provides a safe, supportive setting where youth can focus on stabilization, personal growth, and skill development. During their stay, youth receive comprehensive evaluations, individualized treatment, and guidance to help them build healthy coping strategies and positive life skills. Educational services, therapeutic interventions, structured activities, and recreational opportunities are integrated into each youth’s daily experience.
Every treatment plan is customized to reflect unique strengths, challenges, and goals of each youth. We involve families throughout the process, ensuring continuity of care and a thoughtful transition plan beyond treatment. Eugene offers intensive support through high staff-to-youth ratios, 24-hour nursing services, psychiatric care, therapeutic recreation, and a variety of evidence-based clinical services tailored to each youth’s needs.
Through compassionate care, meaningful relationships, and individualized support, our team of experts empowers youth to build resilience, renew hope, and create a pathway toward a brighter future.
Restorative services
Ages 12-17 Males and females
The restorative services program supports youth ages 12-17 who are involved in juvenile justice system and require an assessment of their ability to move forward with legal proceedings. Through a structured, evidence-based approach, a team of forensic psychologists and behavioral health professionals works closely with youth each week to build understanding, develop skills, and support their engagement in the legal process.
Youth are referred to the program by the State of Oregon and may participate while at a residential or community-based setting. Nexus is proud to provide specialized care, helping address a critical need ensuring youth have access to the support, education, and resources they need.
