Program snapshot

  • Located at the Hennepin County Behavioral Health Center in Minneapolis, MN
  • Serves youth ages 8-17 with complex behavioral health needs
  • Up to 30-day stays
  • Goal is to provide stabilization for youth returning to their community or who are waiting for appropriate longer-term solutions
  • Hennepin County screening team determines eligibility; youth must screen positive for mental health crisis

With 13 beds the Hennepin Youth Stabilization Center provides a family and youth-centered approach to service. We provide 24/7 intensive staffing in a secured facility, where youth receive culturally responsive, family-centered, and trauma-informed care. 

Upon arrival each youth receives a crisis assessment, treatment, care coordination and transition planning. The center is designed for stays of up to 30 days. While in our care, Nexus will partner with families, case workers, and key stakeholders to provide an individualized array of services including onsite education, individual, family, and expressive therapy, daily programming, and recreation services.

Who we serve

We serve youth with complex behavioral health needs. Youth must be referred through a Case Manager to the Hennepin County screening team and must screen positive for a mental health crisis.

Youth must be referred through their county case manager. The Hennepin County screening team determines a youth’s eligibility to receive services. Youth must screen positive for mental health crisis (Minn. Statutes 256B.0624, subd. 3).
