One of the first steps of transitioning out of a Nexus residential treatment program is often to a group home program, which offers staff supervision with individualized coaching and mentoring. Our group home in Onarga provides males ages 14-17.5 a safe, supervised setting that supports them as they practice and implement their new, positive skills.
Nexus employees work side by side with youth in character growth, emotional healing, and social maturity while confronting the core issues that resulted in their placement. Effective and positive behavior techniques are incorporated to promote healing in addition to addressing underlying trauma and mental health needs.
Individual treatment plans are developed for each youth based on specific critical goals that are defined by the placing agency. A youth’s social development is an important life-long process. Treatment plans are an integral part of the program that include strategies for acceptable behaviors that can be used throughout their lifetime. Monthly staff meetings are held to review measurable progress made toward each goal, transitional needs, and reunification planning. Our goal is to equip our youth with the skills to make it on their own and live socially responsible lifestyles.

“Nexus showed me that there is hope for a great life, the staff showed me that people really do care.” – Youth

For professionals and referral partners
Most youth in our group home programs have successfully completed another Nexus-based treatment program, but referrals for youth outside of our network will be considered on a case-by-case basis.
