Nexus Family Healing Treatment Foster Care (TFC) provides highly individualized, wrap around care for youth with complex needs and their families in the foster care system. The TFC program provides embedded mental health resources, case management, skills coaching, parent partnership, and aftercare support with the youth, their biological family, and their foster family.
Treatment Foster Care FAQs
How is this different from regular foster care?
Treatment foster care is designed to wrap supports and services around the child who has a diagnosed mental or behavioral health concern and meets the criteria to qualify for treatment level care. These services include intensive in-home therapy and skills for the child, the permanency home and foster home.
Where and how are youth placed?
Youth are placed in treatment level foster homes through a matching process coordinated by the referral manager in consultation with the treatment team.
Who determines the youth needs treatment level of care?
The youth will be assessed through Maximus Ascend, a 3rd party assessment agency. The initial application for treatment foster care starts with the custodial agent submitting the necessary documentation for assessment. Once approved, a child is given 180 days of treatment foster care, with the ability to receive ongoing TFC with a reassessment done to extend services.
How long can a youth receive services?
The initial approval for TFC is 180 days. Treatment teams will work together to make the determination if additional time is required to achieve the goals of treatment and reach either reunification or permanency. If additional time is advised, a reassessment will be conducted with additional days being authorized by Maximus Ascend.
Host homes are family-style living arrangements for individuals with intellectual or developmental disabilities (ID/DD) in the state of North Dakota. An individual lives with a caring provider in their community surrounded by a circle of support, provided by Nexus Family Healing, instead of living in a group home or institution. This is a voluntary program designed to help individuals return to their home when possible while continuing to receive help and mentorship from their Host Home provider. We are currently seeking homes that are located in Bismarck, Fargo or within 60 miles of either city.

Ensuring Quality Care
Host Homes in North Dakota follows state rules under ND Administrative Code and ND Century Code. This means:
- Providers are licensed have specialized training
- Homes are safe and inspected regularly
- Individuals have personalized support plans
- Providers receive ongoing training and support
- 24/7 help is available for emergencies
The Chafee Program
(Ages 14+)
The mission of the Chafee Program is to ensure that youth involved in the foster care system receive services and support which will enable them to successfully transition to living independently. The Chafee Program is provided by Nexus-PATH in partnership with the ND Department of Human Services and is one way to assist custodians in meeting the foster care policy requirement of assessing and addressing independent living needs of foster youth.
The Chafee Program serves youth ages 14 and older who are or were in foster care get access to voluntary programs for education, housing, transportation, and other resources.
Supervised Independent Living
(Ages 18-21)
The Nexus-PATH Supervised Independent Living (SIL) Program provides programming as an approved living arrangement under North Dakota’s 18+ Continued Care. Through our voluntary service, young adults between 18-21 years of age can live on their own while continuing to receive support services to become more independent and self-sufficient. Young adults can live independently without 24-hour adult supervision. This allows them to experience more responsibilities while receiving assistance in accessing community resources, health education, academic support, career preparation and employment support, and guidance in healthy relationships. They may be provided with a furnished apartment, school-based housing, or may provide their own living setting.
